Note: Two New Yorkers are speaking in this clip, but the pronunciation is often "How ya doin'?" in many regions of the USA.

very well = fine!
very and very well

adjective = to a high degree
Note that "very well" translates well into all languages, while "How are you?" does not translate correctly to languages which use a different verb for the same question.
